Referral Forms

 

All the forms include the Contact Centre's "Parent's Agreement" which sets out the ground rules.  Every user of the Centre must sign and return the "Parent's Agreement" before contact can start. At Walton Child Contact Centre we also request that you make a visit to the Contact Centre, on a Saturday afternoon, to meet with the Centre Coordinator before contact can start.

If you don't have a solicitor and you want to use the Contact Centre, both parents need to complete a form. 

The parent who lives with the child(ren) is called the Resident Parent and should click this link for the form.

We require a referral form in respect of both parents, either from their legal representative or directly from them.
